All families are required to submit the Family Information Form, whether participating as a member or non-member. It contains important health and current emergency information. This form can be submitted with dues if joining, or without dues if simply enrolling as a class participant.

The Family Information Form can be submitted via email, faxed to 312-664-9364, or feel free to drop off in person!
